Small Dogs Only Daycare Enrollment Form
By filling out and submitting this form, you agree to the following terms and conditions:
Your dog will be kept up-to-date on required vaccinations, be kept on a flea and parasite preventative, and have yearly veterinary examinations. Dogs over six months of age are required to be spayed or neutered to join daycare.
Dogs should not be brought to daycare when they are exhibiting signs of an illness that could infect other dogs. Dogs will be checked for fleas, and if fleas or other parasites are found, your dog will be removed from daycare and treated at your expense.
Dogs must be kept on a leash or in a carrier when entering and exiting our facilities.
Although we make every effort to prevent injury, due to the nature of group play, injuries may occur. In the event of your dog experiencing injury or illness, we will contact you for authorization to treat your pet. In emergency situations, we will provide immediate care to stabilize and make every attempt to contact you before proceeding. In the event that we are unable to reach you, you will be responsible for any charges to stabilize your dog.
You will not hold East Towne Pet Clinic liable for injury or illness that occurs during daycare.
Your dog may be temporarily or permanently removed from group play if he or she exhibits inappropriate or aggressive behavior. Dogs who have previously harmed another person or animal are not permitted.
Dogs who engage in group play sessions may get dirty or require more frequent grooming. Additionally, dogs not accustomed to prolonged play times may experience sore muscles, sore paws or fatigue, and may exhibit excessive drinking or eating upon arriving at home.
Payment is due in full at the end of each day. A late fee or overnight boarding fees will apply if you fail to pick your dog up by closing time.
You allow your dog to be photographed and videotaped, and that such videos and photographs are the sole property of East Towne Pet Clinic and may be used or distributed without usage fees.
